To complete the form you will need your Vaccine Pass, date of birth and an email address (you can use your own or your club email)
This form is only for registered athletes and volunteers.

Terms and Conditions

  • After verifying your Vaccine Pass you will still need to carry your Vaccine Pass to enter venues for training and Special Olympics events.
  • Special Olympics New Zealand will not collect, use or disclose any personal information submitted for any other purpose other then to confirm vaccination status under the COVID-19 Public Health Response Act 2020.
  • Under the Privacy Act 2020 Special Olympics New Zealand and Sporty will ensure there are reasonable safeguards in place to prevent the loss, misuse or disclosure of personal information.

Help uploading your vaccine pass

Upload your vaccine pass from your phone or PC

  1. Take a screenshot of your vaccine pass on your phone, or save your vaccine pass PDF to your PC or phone.
  2. Click the upload PDF or Screenshot and select your vaccine pass
  3. Your vaccine pass details will show up with “verified” if successful
  4. Click ok and scroll down the form to click “Submit” to verify your vaccine pass with SONZ.

Scan your vaccine pass with your camera

  1. Click “Scan QR Code from Camera” You may be required to allow camera permissions, please click yes.
  2. Hold your document or device close to the camera to get as much of the QR code in the window as possible, you may need to move it closer or further away before the scanner will detect it.
  3. Your vaccine pass details will show up with “verified” if successful
  4. Click ok and scroll down the form to click “Submit” to verify your vaccine pass with SONZ.
