SONZ has implemented a Customer Relationship Management (CRM) system called Sporty. As of Monday 7 October 2024, new registration forms and processes are in place for Special Olympics Athletes and Volunteers/Unified Partners.
These forms are in place for all Clubs – not just those who are piloting the CRM system.
There are four registration forms – explanations of how these are used are outlined below. Please note the first step for each form is to select the relevant Special Olympics Club – this is to ensure the information is populated to the correct Club within Sporty.
New Athlete Registration Form
- This is a full registration form that captures an athlete’s contact details, emergency contact details, participating sports, code of conduct etc.
- This form links to the Medical Declaration Form, which is where the Medical Certificate needs to be uploaded – see below.
- Both the Athlete Registration Form and the Medical Declaration need to be completed for a new Athlete.
New Volunteer/Unified Partner Registration Form
- This is a full registration form that captures volunteer’s contact details, emergency contact details, what volunteer role they are undertaking etc.
- This form links to the Police Vetting Form, which is where the volunteer enters their details and consent for Police Vetting – see below.
- Both the Volunteer/Unified Partner Registration Form and the Police Vetting Form need to be completed for a new Volunteer/Unified Partner.
Athlete Medical Declaration
- This form is where the Medical Certificate is uploaded.
- It is separate from the full registration form and completed on initial registration and then every 4 years.
- When just renewing a medical for an existing athlete, you do not complete any other registration form.
Police Vetting Application
- This online form replaces the paper Police Vetting forms that SONZ has previously used; the volunteer enters the equivalent details directly into the online fields.
- It is separate from the full registration form and completed on initial registration every 3 years.
- No paper forms or copies of ID are required to be submitted to SONZ.
- A Club representative is required to verify the volunteer’s ID.

Athlete Medical Certificate
The SONZ Athlete Medical Certificate has been updated following review and feedback from clubs, and consultation with health practitioners. Both this new and the “old” SONZ Medical Certificate (dated January 2013) will continue to be accepted until the end of 2024. From January 2025, the new one (dated September 2024) needs to be used.
